In this article, Lina Mooren talks about the Digital Health Centre (DHC), a regional initiative to support patients with chronic conditions in managing their health from home. They use Patient Journey App, which shares educational information from both the medical and social domains, allows home monitoring, and facilitates health consultations via video calls with telecoaches.

Lina explains in the article why they chose Patient Journey App and the effect it has on patients:
"Patient Journey App is an intuitive app that they can easily use. Recently, I heard a patient say: 'Since the DHC, the disease no longer controls me, but I control my disease.' That is, of course, the ultimate goal."
